How do I run the scan to have only those stocks listed which have "FRESH" buy/sell signal/arrow?
For the scan you will see some stocks with brand new signals and some with prior signals on the charts. With TOS there is no way to pinpoint to "fresh" signals I have experimented with the scan and my best option would be scan in the morning then 4 hours later. If a signal was flagged follow up with it the following day to confirm direction. Why this happens is because the scan is real time so I like to wait until the close of the 4 hour bar.

How do I setup alerts in TOS?
Right click scroll down to alerts. Create alert. You will see alerts also on the MarketWatch tab to confirm/adjust your pre- set alerts.
